Gender Affirming Care
ADULTS:
We provide Gender Affirming Hormones for transgender, nonbinary, and gender fluid/non-conforming ADULTS in NE, IA, SD, ND, KS, CO, WA and TX.
We use the informed consent model and follow WPATH guidelines. A therapist letter is not required to start hormones for adults.
Requirements for an appointment:
Capacity to give informed consent
Age of majority in your state (19+ in Nebraska)
Any medical or mental health issues are reasonably well controlled
Your provider will discuss the informed consent process, discuss your gender identity development history, review effects, and possible risk factors for gender affirming hormone therapy prior to writing a prescription.

MINORS:
For minors age 16 and up. We are able to see new minors in person at our Bellevue, NE location ONLY. Follow-up appointments may be done by telehealth.
Requirements for an appointment:
Both parents/guardians must sign consent for hormone therapy. NO EXCEPTIONS unless court documents supplied.
Letter of support from a licensed mental health provider dated within the last 6 months. Please have them fax the letter to us (fax number below) or email it using the contact us button above. The letter must include the following to be accepted:
The client’s general identifying characteristics
Results of the client’s psychosocial assessment, including any diagnoses
The duration of the referring health professional’s relationship with the client, including the type of evaluation and therapy or counseling to date
An explanation that the criteria for hormone therapy have been met, and a brief description of the clinical rationale for supporting the client’s request for hormone therapy
A statement about the fact that informed consent has been obtained from the patient
A statement that the referring health professional is available for coordination of care and welcomes a phone call to establish this
