Leslie Dvorak, MSN, APRN, WHNP-BC she/her
Leslie graduated in 2002 from the University of Nebraska Medical Center with a Master's Degree in Nursing. After retiring from the Air Force she started working at a community based health center where she provided sexual and reproductive healthcare for underserved communities. Leslie is passionate about providing safe, accessible, quality, gender-affirming health care to the LGBTQIA2S+ community as well as those with non-conventional sexual or lifestyle practices including polyamory, Kink and BDSM. She educates medical providers on gender-affirming care as well as contraception, physical exams, intimate partner violence, sexual assault and human trafficking. Leslie is a member of WPATH, GLMA (Health Professionals Advancing LGBTQ Equality), ACOG, and NPWH.
Nurse Case Manager/Owner:
Lindsey (Creekmore) Koch, RN

Lindsey has been a registered nurse for over 20 years. She has experience in multiple fields including gender affirming care and women's health. She is a strong advocate for becoming the best version of ourselves. Anything you need done in the clinic, Lindsey can do it. Need something notarized or an ordained minister, look no further.
Provider: Amy Jacoby-Loftis, LCSW, LMHP

Amy graduated in 2017 from University of Nebraska at Omaha with a Master’s Degree in Social Work and has since obtained her licenses as a Clinical Social Worker and Mental Health Practitioner. Prior to this while serving 12-years in the Air Force, Amy worked as a Medic, providing care in both inpatient and outpatient capacities in Emergency Rooms, on Labor and Delivery, and in both Woman’s Health and Primary Care Clinics. As a Social Worker, Amy’s clinical experience includes providing individual mental health therapy, group therapy, crisis intervention and management, and intensive case management services. Specialized or underserved populations she has experience working with to provide safe, quality, compassionate, gender- and sexuality-affirming, trauma-informed care to include the LGBTQIA2S+, non-conventional sexual or lifestyle practicing communities, individuals with co-occurring mental health and substance use, intimate partner violence, sexual assault or human trafficking needs, incarcerated and criminal justice system-involved individuals, military Veterans and their families, and individuals with severe and persistent mental illnesses. Amy welcomes her clients to make today better than yesterday, and she encourages them to make tomorrow better than today. She enjoys providing clients clinically therapeutic opportunities to learn, grow, and heal while they process the experiences that may be holding them back from living their truest, most fulfilling lives.
Clinic Assistant Administrator/Medical Assistant: Rachelle Carr, MA

Rachelle has been a Medical Assistant for 20 years, along with her in patient case management she has experience in HIV Care including gender affirming care. She is a devoted ally and advocate, along with assisting vulnerable patients. Rachelle is a notary, telehealth expert and is here to assist you to “Just Be You”
